Abstrakt     BACKGROUND: Chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema is characterized by progresive airway obstruction that is not fully reversible with the administration of bronchodilator drugs. Computed tomography (CT) is an established tool for in vivo assessment of pulmonary emphysema, which spears as areas of decreased lung attenuation. Despite numerous investigations, the morphologic evaluation of COPD by HRCT has not yet been fully standardized. Expiratory CT can reveal air trapping in the lungs, but which method is the most accurate in predicting pulmonary function tests (PFT) is not yet known. AIMS: We tried to find out correaltion between attenuation measurements of the lung on inspiratory and expiratory scans and pulmonary function tests (PFT) in patients with COPD. Influence of regional distribution of emphysema on PFT and value of paired inspiratory/expiratory CT scans in prediction of PFT in patients with COPD was also evaluated. SUBJECTS AND METHODS: Thirtytwo current or ex smokers with COPD underwent inspiratory and expiratory CT scans and PFTs. Three scan pairs (in the upper, middle and lower lung fields) were evaluated by densito- and planimetry using dedicated software. Two emphysema indexes (EI -950 and EI -900), mean lung density (MLD) and cross-sectional lung area were measured. Change in cross-sectional lung area at inspiration and expiration (area reduction score-ARS), ratio of emphysema indexes at inspiration and expiration and ratio and difference between MLD at inspiration and expiration were calculated.
