Abstrakt     Background: Pregnancy and livebirth rates following in vitro fertilization-embryo transfer (IVF-ET) depend on embryo quality and the number of transferred embryos, but mostly on the mechanisms involved in embryo implantation. The aim of this prospective randomized comparative study was to compare the effect of two different methods of ovarian stimulation in IVF-ET on certain morphometric and biochemical endometrial factors, affecting embryo implantation. Methods: In the study we enrolled 100 randomly selected infertile women. According to the type of ovarian stimulation, they were randomly divided in two groups: 50 women received the gonadotropin releasing hormone (GnRH) antagonist Cetrotide (group C), and 50 the GnRH agonist Suprefact0 (group S). The inclusion criteria were: age < 38 years, normal body weight, regular menstrual cycle, no more than 3 previous failed IVF attempts and the presence of motile sperm after semen preparation. In all enrolled women, IVF-ET and endometrial biopsy during oocyte retrieval were performed. Morphometric (histological endometrial dating, pinopodes) and biochemical (interleukin-1alpha , interleukin-1beta, interleukin-1 receptor antagonist, leukemia inhibiting factor, glycodelin-A and mucin MUCl) factors were determined in endometrial biopsy samples and compared between the groups. Results:Histological endometrial dating (in days) showed advanced maturation in both groups, significantly less advanced in group C (median +l day) than in group S (median + 2 days) (p = 0.025). Pinopodes were found present in 40.9% of women in group C, and in 12.5% of women in group S yielding a significant difference (p = 0.044). (Abstract truncated at 2000 characters)
