Abstrakt     Chronic pain represents a mixture of pathophysiologic mechanisms, a complex assortment of spontaneous and elicited pain states, and a somewhat unpredictable response to analgesics and is still present after 3 to 6 month as primary reason (disease, trauma) had already extinct. It is a burden for a patient as lowers a quality of life, for a physician as therapeutic problem and for a society as financial burden. Agents with Little activity in the treatment of acute pain, such as antidepressants, antiepileptics, and i.v. administered local anesthetics, are initiated in many patients and have significant long-term efficacy in some patients with chronic pain. When all other procedures or treatment are exploited to treat moderate to severe chronic pain we use opiates. In order to achieve are best approach to treat nonmalignant chronic pain with opiates Slovene society for pain had written recommendations for physicians who deal with those patients. They enable the physician to chose the appropriate patient individually and best drug for one, best way how to start and how to end treatment. They also enroll a patient as an active part with informed consent and responsibility for the whole process of treatment.
