Abstrakt     The early diagnosis of human immunodeficiency virus (HIV) infection in exposed infants is difficult because of the passive transfer of maternal anti-HIV antibodies that are present in infants up to 18 months of age. Therefore, the diagnosis of HIV infection among young infants now relies exclusively on molecular tests detecting HIV proviral DNA or HIV RNA, mainly by polymerase chain reaction (PCR). Molecular testing should be performed in newborns two days after delivery, between 1 and 2 months of age, and between 3 and 6 months of age. A perinatally HIV-exposed infant is determined to be HIV infected when results of HIV molecular test from infant's blood samples obtained at two different times are positive. If the results of molecular testing are persistently negative until 4 months of age, an exposed child is considered HIV negative. HIV infection is ruled out in children 18 months of age or older who have negative anti-HIV serology and negative HIV molecular tests results.
Izvleček     Razlikovanje okužbe s humanim virusom imunske pomanjkljivosti (HIV) od pasivnega prenosa anti-HIV protiteles pri otrocih, rojenih materam, okuženim s HIV, je eden največjih problemov v diagnostiki okužbe s HIV. Zgodnja diagnoza vertikalnega prenosa okužbe s HIV temelji predvsem na določitvi RNK HIV ali DNK HIV z metodo verižne reakcije s polimerazo (PCR). Otroke mater, okuženih s HIV, je priporočljivo testirati na prisotnost RNK HIV/DNK HIV v prvih 48 urah po rojstvu, med prvim in drugim mesecem starosti ter med tretjim in šestim mesecem starosti. Okužbo s HIV potrjujeta najmanj dva pozitivna rezultata iz vzorcev, odvzetih v različnih obdobjih. V primeru ponovljivo negativnih rezultatov molekularnih testov do vsaj četrtega meseca starosti nedojenega otroka smatramo za neokuženega. Okužbo s HIV lahko dokončno izključimo, če je rezultat serološkega in molekularnega testiranja pri otrocih, starih 18 mesecev ali več, negativen.
