Abstrakt     Multidrug resistant (MDR) Mycobacterium tuberculosis strains are threatening control of tuberculosis (TB) in the world. Rapid diagnosis of resistance in TB patients is essential for adequate treatment and optimal control of the disease. Resistance to rifampicin (RMP) results in 95% cases from nucleotide changes in the gene encoding the beta-subunit of the RNA polymerase (rpoB). The main objective of this study was to evaluate a commercially available line probe assay INNO-LiPA Rif.TB in comparison with conventional microbiologiacal methods and to determine the frequency of mutations in rpoB gene in rifampicin-resistant M. tuberculosis isolates. The PCR-reverse hybridization assay was used on 46 sensitive and 31 resistant isolates of M. tubeuculosis. The sensitivity of assay was 98,7% (76/77). In 16 resistant strains, specific mutation S531L was detected, in 6 specific mutation D516V was detected and in 3 resistant strains specific mutation H526D was detected In 5 strains mutations were present but their exact nature was not determined by the assay (deltaS patterns). In one case, two mutations (deltaS1+deltaS2) were observed. Mutation causing H526Y was not detected. All sensitive strains had WT pattern while among resistant isolates, WT pattern was obtained in one case, possibly due to the absence of mutations in the hot spot region of the rpoB gene. Assay accuracy was 98,7% (75/76). In contrast to sensitivity testing to antituberculous drugs by proportion method on solid LJ media and by the BACTEC MGIT 960 system, INNO-LiPA Rif.TB test is a fast and convenient method for detection of resistance to RMP on culture isolates. Because false positive and false negative results may appear, INNO-LiPA Rif.TB test alone is not appropriate for diagnostic procedure. This molecular test can only be used as on additional method for conventional susceptibility testing.
