Abstrakt     Background: Regional metastases are the main factor defining survival of patients with carcinoma of the oral cavity, the oropharynx and the larynx. Regional metastases are best treated with dissection of lymphatic nodes of the neck. We were interested in treatment results of combined MRND type III and post-operative radiation in patients with cancer of the oral cavity, the oropharynx and the larynx. We also wanted to analyse a pattern of regional recurrency and patient survival, along with the influence of unfavourable factors on disease recurrency and patient survival. Patients and methods: A retrospective overview of patients treated with the same protocol included 72 patients with squamous cell carcinoma of the oral cavity, 57 patients with oropharynx carcinoma and 67 patients with larynx carcinoma. Patients in stage III and IV of disease were postoperatively irradiated. Patient survival has been analysed together with the pattern of regional and local recurrency, the dependence of survival and regional recurrency on unfavourable factors. The success of MRND type III in treatment of regional metastases has also been evaluated. The analysed unfavourable factors were the following: the size of lymphatic node over 20mm, extracapsular spread of metastasis, perivascular and perineural spread, more than two metastatic lymphatic nodes. Additional unfavourable factors were included; bone invasion in the oral cavity carcinoma, invasion of the pterygomandibular space in the oropharyngeal carcinoma and pre-operative tracheotomy and/or metastases in VI and IV region of the neck in laryngeal carcinoma. Results: Overall five year survival of patients with oral cavity carcinoma was 64%, wheras disease specific survival was 86,3%. (Abstract truncated at 2000 characters)
