Abstrakt     Objective. Platelet activation and its role in the development and outcome of acute coronary syndromes (ACS) is widely recognized, but it is unclear whether platelets are selectively activated in the coronary artery with the culprit stenotic lesion. The purpose of this study was to explore this prediction. Methods. In vitro closure time test (CT, kol/ADP and kol/Epi) and platelet P-selectin expression were measured in 14 patients with either STelevation or non-ST-elevation acute myocardial infarction (AMI), 8 patients with stable angina pectoris (AP) and 10 control patients with a normal coronary angiogram (C). Blood samples were taken from the right and the left coronary artery, femoral vein and femoral artery and additionally from the coronary artery at the site of the culprit lesion in AMI group. Results. There was no difference in values of either CT or P-selectin between coronary artery and systemic blood in any of the three studied groups. Furthermore, no difference between samples obtained at the culprit lesion and the unaffected coronary artery were obtained. Overall, CT was significantly shorter in AMI group compared to AP and C group (p<0,001) and platelet P-selectin expression was significantly lower in the C group compared to AMI and AP group (p<0,001). Conclusion. Our results revealed no additional local activation of platelets at the culprit lesion in patients with AMI or in coronary arteries in patients with AP. Thus, it seems that systemic activation of platelets rather than local activation occurs during an acute coronary event. Certainly activated platelets contribute to the pathophysiologic processes at the culprit lesion.
