Abstrakt     Calcific aortic stenosis is the most frequent expression of valvular heart disease in the Western world. It is a progressive, degenerative, atherosclerosis-like process that involves aortic valve with increasing prevalence as the population ages. Surgical aortic valve replacement is the treatment of choice for patients with severe, symptomatic aortic stenosis, but it has limitations in old patients with high perioperative risk. Balloon aortic valvuloplasty (BAV) is a less invasive catheter-based percutaneous option which is performed in patients with a prohibitive surgical risk. Despite it offers acute symptoms relief and better quality of life after dilatation, in the past these procedures fell from favor secondary to suboptimal initial results, high rate of restenosis and lack of impact on long term survival. In the last few years we have observed a renaissance of BAV along with the development of percutaneous aortic valve implantation methods. Clinical and pre-clinical studies are looking for new solutions for preventing valve restenosis and delaying valve mineralization after BAV. In the future the use of BAV will depend on the effectiveness of these methods.
