Abstrakt     Surgically skilled interventional nephrologists are capable to create not only typical radiocephalie arteriovenous fistula but also all kinds of native arteriovenous fistulae and grafts as well as to perform surgical salvage procedures for thrombosed arteriovenous fistulas and grafts. Interventional nephrologists can therefore provide complete care over hemodialysis patients, being the most important factor of patient's high quality hemodialysis treatment and his well-being. In Slovenia there is a long lasting tradition of creation of arteriovenous fistulas and grafts as well as performing salvage surgical procedures when thrombosis occurred by nephrologists. For the last several years the patients from all the country with acute thrombosis of arteriovenous fistula or grafi have been able to be admitted immediately in our Dialysis center for thrombectomy. In the group of 111 thrombosed native arteriovenous fistulas thrombectomy or thrombectomy with creation of a new anastomosis was performed and immediate success rate was 93.8%.One year secondary (assisted) patency was 73%. In the group or 27 thrombosis of polytetrafluoroethilene grafts thrombectomy (performed by nephrologist) was followed by fistulography and transluminal angioplasty, performed by radiologists. The immediate success rate was 63% and one year secondary (assisted) patency was 48%. There are data in the literature confirming superiority of surgery over endovascular procedure s which support our dinical practice and actually encourage salvage procedures to be performed by interventional nephrologists.
