Abstrakt		Aortic stenosis is a common heart valve disease with increasing incidence due to the aging of population. Although surgery provides excellent and durable results in most patients, advanced age and comorbidities may pose great risk. Therefore mini mally invasive techniques have emerged as an alternative choice of treatment for these very high-risk patients, offering them not only the symptomatic relief but long-term survival. Transapieal aortie valve replacement is a hybrid catheter-based method for implatation of the prosthetic valve for patients in whom open heart surgery or percutaneous transfeomoral procedure is not feasible or for patients who pose unacceptable risks. Case report. We describe her a case of a 77-year-old woman with severe, symptomatie aortic stenosis with concurrent malignant ilInes, that was refused for surgieal aortic valve replacement. Due to worsening of her symptoms and a combination of aortic stenosis and insufficiency the transapieal aortie valve implantation was preformed. After the procedure she improved dinically and objectively. Condusions. Our case shows that there are additional therapeutie options for high risk patients with severe aortie stenosis. In our opiniof1 the transapical aortie valve replacement presents an acceptable alternative treatment of aortic stenosis, especially when combined with aortie insufficiency, in high risk patients. Prercutaneous implantation of aortie valve demands for teamwork of cardiologist, cardiac surgeon and anesthesiologist. This combined effort together with dinieal data will provide new dinieal indieations for percutaneous aortic valve implantation.
