Abstrakt     Successful resuscitation from cardiac arrest is in part contingent on preservation of left ventricular distensibility; a feature that enables chest compression to generate higher forward blood flows and higher coronary and cerebral perfusion pressures consequent to preservation of left ventricular preload. Gazmuri group has reported that left ventricular distensibility can be preserved during resuscitation by interventions that protect mitochondrial bioenergetic function. Ischemia and reperfusion is associated with injury caused in part by the production of reactive oxygen species (ROS). Vitamin-C (ascorbic acid, AA) is a potent antioxidant that is uptaken by cardiomyocytes either via the sodium-dependent co-transporter, SVCT2 or via the facilitative glucose transporter, GLUT1. Yet, to be uptakenby GLUT1, AA must be in its reduced form, dehydroascorbate (DHA). The aim of this article is presentation elementary characteristics of experimental study about vitamin-C and ROS in cardiopulmonary resuscitation and up-to-date informations about function of ROS in metabolism of cell.
Izvleček     Uspešnost kardiopulmonalnega oživljanja (KPO) je deloma odvisna od ohranjanja podajnosti levega prekata. Ohranjena podajnost (in s tem ohranjena polnitev) levega prekata omogoča med zunanjo masažo srca boljši krvni obtok in s tem večji pretok skozi venčne in možganske arterije. Z zaščito in ohranjenim delovanjem mitohondrijev lahko ohranimo podajnost levega prekata. Ishemija in reperfuzija povzročita reperfuzijsko poškodbo, ki zmanjša podajnost levega prekata in je deloma posledica nastanka prostih kisikovih radikalov (reactive oxygen species, ROS). Vitamin-C (askorbinska kislina, AK) je močen antioksidant, ki ga kardiomiociti lahko prevzamejo preko od natrija odvisnega kotransporterja (sodium-ascorbate co-transporter, SVCT2) ali preko transporterja glukoze (facilitative glucose transporter, GLUT1). Za prevzem preko GLUT1 mora biti vitamin-C v reducirani formi - dehidroaskorbat (DHA). V sodelovanju z Resuscitation Institute Rosalind Franklin University in Centra za nujno medicinsko pomoč ZD Adolfa Drolca je na osnovi teh dejstev nastala hipoteza in osnova za eksperimentalno študijo o vlogi ROS-a in vitamina C pri KPO. Namen prispevka je prikazati osnove te študije ter najnovejša dognanja o vlogi ROS-a pri metabolizmu celice.
