Abstrakt     Dry eye or keratoconjunctivitis sicca (KCS) is recognized as a disturbance of the lacrimal functional unit, resulting in tear film hyperosmolarity and ocular surface inflammation. Due to different definitions of dry eye the prevalence ranges from 5 % to over 35 % at various ages. The ethiopatogenetic classes of dry eye are aqueous tear-deficient dry eye and evaporative dry eye. The diagnosis can be made by thorough medical history and different diagnostic tests: tear break-up time test, ocular surface dye staining, Schirmer's test, expression of Meibomian glands, and corneal sensitivity test. These tests should be performed in this sequence. Recently published guidelines propose a severity classification based on clinical signs and symptoms, with treatment recommendations according to severity level. Due to new knowledge about the role of inflammation in pathogenesis of dry eye the treatment is directed from lubricant to anti-inflammatory therapy.
