Abstrakt     Background: One of the most revolutionary developments in vitreoretinal surgery over the last several years is 25-gauge transconjunctival sutureless vitrectomy. Compared with standard 20-gauge pars plana vitrectomy, the smaller 25-gauge system allows for self-sealing transconjunctival pars plana sclerotomies. This leads to decreased surgical time, less postoperative inflamation, and faster postoperative recovery. Ideally it is used for vitrectomy for macular hole, macular pucker, vitreous haemorrhage and macular edema. In this article we describe historical moments in development of this technique, benefits, possible complications and experience at our clinic. Conclusions: In spite of all possible complications linked to this method (postoperative hypotony, higher risk of endophthalmitis, lower infusion and aspiration rates, lack of various instruments) it has become one of the standard methods in vitreoretinal surgery.
