Abstrakt     Angiography is a golden standard for coronary artery lumen anatomy visualisation and detection of changes that narrow the lumen and guiding percutaneous coronary intervention (PCI). Angiography is enough when changes found are significant by angiography criteria (>70 ` stenosis). However in some cases angiography is not enough since it provides only an indirect insight into the process in the vessel wall. For advanced diagnostics of coronary artery disease advanced imaging techniques should be used, such as intravascular ultrasound (IVUS), virtuat histology (IVUS-VH) and optical coherence tomography (OCT). For physiological assessment of stenoses fractional Flow reserve (FFR) should be used. IVUS allows for an accurate qualitative and quantitative assessment of vessel lumen, vessel wall and plaque on cross-sectional and longitudinal view. IVUS-VH allows with the additional analysis of IVUS acquired sygnal color representation of 4 tissues within the atherosclerotic plaque (fibrosis, fatty tissue, necrosis, calcium) and therefore the analysis and assessement of the plaque in terms of stability and vulnerability. OCT has far greater resolution then IVUS and allows an even more accurate insight into the composition of the intima, plaque, restenosis, thrombi, etc. FFR is a method for physiologic assessement of stenoses. It is a gradient between pressure measured distal from the stenosis by pressure wire and pressure measured in the aorta by guide catheter in stabfe conditions of maximal hyperemia of the microcirculation which is established by intravenous adenosin infusion.
