Abstrakt     Methods: Clinical and laboratory data and outcome of invasive S. aureus infections in children <15 years of age, hospitalised at the Department of infectious Diseases in Ljubljana from 1st June 2006 to 31st December 2010 have been reviewed. Staphylococcal isolates were tested for the presence of Panton-Valentine leukocidin (PVL). Also, medical charts of children with culture-negative osteomyelitis treated in the same period were analysed.Results: In a 4.5-year period, 19 children with invasive S. aureus infection were seen, 17 had osteoarticular infection, one had prosthetic valve endocarditis and one had pacemaker infection. Mean duration of antibiotic therapy was 48 days, surgical treatment was required in 11/19 children. In two children chronic osteomyelitis developed. All staphylococcal isolates were methicillin-sensitive, 4 were PVL-positive. Three children with PVL-positive infections developed necrotising pneumonia with spontaneus pneumothoraces, the remaining child had locally invasive osteomyelitis with progression into chronicity. Children with culture-negative osteomyelitis required shorter treatment with excellent outcome.Conclusion: The majority of invasive staphylococcal infections affect the osteoarticular system and in spite of prolonged antibiotic treatment do not always have a favourable outcome. PVL-positive isolates have a more severe disease course.
