Abstrakt     The lactic acid that circulates in the human body is the product of the anaerobic metabolism of glucose. Lactate is always produced by reduction of pyruvate and is always removed by reversal of this process (Cori cycle). A defect in any of these pathways may lead to inadequate removal of pyruvate and lactate from the circulation, and lactic acidaemia or acidosis results. Inborn errors of metabolism and/or hypoxia in the new-born and infant period as the cause for lactic acidosis are not so infrequent, and may present common neonatal problems such as systemic infection, congenital heart disease, intracranial haemorrhage and so on. The classification, clinical picture, step-by-step laboratory clues, and the biochemically defined inborn errors of metabolism with lactic acidaemia or acidosis are briefly presented.
