Abstrakt | The purpose of the present study was to evaluate the possible influence of insulin on the heart muscle mass in hypertensive patients. A total number of 46 hypertensives (systolic blood pressure 150+-11, diastolic blood pressure 97+-6 mm Hg) was included in the study (mean age 37.5+-7 years). They were all non diabetic. 21 healthy volunteers (systolic blood pressure 124+-7 years). They were all non diabetic. 21 healthy volunteers (systolic blood pressure 124+-6, diastolic blood pressure 81+-4 mm Hg) served as control group (mean age 33+-4 years). Only men were included. We measured blood pressure and calculated Quetelets (Q) index. Fasting insulin, glucose and lipid concentration were determined. Fasting insulin concentration was determined by radioimunoassay. A bidimensional echocardiographic analysis was performed on every enrolled subject. Measurements of posterior wall thickness, interventricular septal thickness and left ventricular diastolic dimensions were made. Left ventricular mass was calculated according to the method of Devereux. Fasting insulin concentration in the group of hypertensive patients was 12.3+-14.7 microU/ml, in control group 5.6+-5.1 microU/ml (p less th. 0.05). Patients with hypertension had higher insulin/glucose index (2.15ž+-2.6) than normotensive volunteers (1.11+-1.13) (p less th. 0.02). Left ventricular mass in the group of hypertensives was 237+-87 g, in the control gorup 188+-45 g (p less th. 0.02). Q index was higher in the group of hypertensives (2.61+-0.38 g/cm2) than in the control group (2.37+-0.12 g/cm2) (p less th. 0.01). Statistical analyses were performed using the multiple linear regression method. We found a positive correlation between left ventricular mass and fasting insulin concetration in the group of hypertensives, but only due to one subject which had a strong influence on the regression line.
