Abstrakt     The study discusses the thrombolytic treatment of acute and subacute arterial occlusions of the extremities in 21 patients, whose thrombi were enriched with exogenous lys-plasminogen before treatment. Immidiately after the arteriography, each patient recieved 7.5 mg of plasminogen by intraarterial catheter. After this, they were continously treated with 3750 IU/hour of streptokinase. At the same time 450 IU/hour of heparin were administred by the external lumen of the double coaxial catheter so that heparin poured over the distal part of catheter. The treatment was so successful in 11 patients and failed in 5 patients, who needed amputation of the leg. In 2 patients partial lysis was observed. Three times therapy was discontinoued. In the patients that were treated successfully, euglobulin lysis was significantly shorter after 24 hours of treatment compared to the pretreatment values. Additionaly, significantly lower plasminogen levels were determined locally and systematically in these patients. The results suggested systemic activation of fibrinolysis.
