Abstrakt | Na zastrupitev z ogljikovim monoksidom (CO) v urgentni medicini največkrat pomislimo ob povedani anamnezi, a jo moramo izključiti tudi ob vseh motnjah zavesti neznanega vzroka. Pri tem nam je lahko v veliko pomoč takojšnje neinvazivno določanje CO na osnovi pulzne spektrometrije, ki ga omogočajo sodobni aparati za kontinuirano monitoriranje bolnikov. Zgodaj in jasno postavljena diagnoza bistveno vpliva na potek oskrbe in hitrost pričetka vzročnega zdravljenja. Prikaz primera obravnava predbolnišnično oskrbo bolnika, ki je bil najden neodziven, v travi na odprtem v naselju, zaudarjal je po alkoholu. Ob stabilnih vitalnih parametrih in, z izjemo motnje zavesti, normalnem kliničnem statusu, brez znakov za poškodbe, je izstopala le samodejno izmerjena visoka vrednost karboksihemoglobina. Vsled takojšnjega vzročnega zdravljenja, smo bolnika predali v internistično urgentno ambulanto z bistveno nižjo vrednostjo CO v krvi. Že naslednji dan je bil brez posledic odpuščen v domačo oskrbo. Vzrok zastrupitve z ogljikovim monoksidom je bilo intenzivno kajenje večih ljudi v majhnem, zaprtem prostoru, ob pridruženem akutnem alkoholnem opoju.Diagnosis of carbon monoxide (CO) poisoning is mainly based on compatible history, but should also be considered as a differential diagnosis in altered mental status of unknown etiology. Innovative monitors integrate noninvasive pulse CO-oximeters, capable of spectroscopic measurement of carboxyhemoglobin,which may be useful diagnostic tool in the prehospital setting. Appropriate initial treatment is based on accurate early diagsnosis. This case report discusses the prehospital treatment of a man, found lying unresponsive outside on grass smelling of alcohol. His vital signs were stable, he had no signs of injury, however a high level of carboxyhemoglobin was measured by CO-oximetry. The immediate treatment was started already in the field and the patient was brought to the Internal Emergency Department with significantly lower value of carboxyhemoglobin. He was discharged from the hospital without consequences the next day. The cause of carbon monoxide intoxication was intensive cigarette smoking in a small enclosed room and concomitant alcohol intoxication. | |
