Abstrakt     Numerous sometimes interrelated conditions are leading to rhabdomyolysis with consequent acutre renal faillure and even death: exertional states, muscle injuries, infections, systemic diseases, circulation disorders, inherited and acquired metabolic, toxic disorders and various drugs. The author present 34-years old male chronic, who went through epileptic seizures, delirium tremens, hypotension and febrile condition and developed crush-syndrome like clinical state with myoglobinuria, acute renal faillure and died. Autopsy revealed acute tubular necrosis with interstitial edema and numerous immunohistologically proven myoglobin casts in the tubules. Skeletal muscles disclosed patchy muscle fibres necroses. The author suggest multifactorial pathogenesis of rhabdomyolysis in the patient induced by: exertional states during epileptic seizures and delirium tremens, febrile state, direct toxic influence of alcohol, and influence of hypotension with hypoxia.
