Abstrakt     We have devised an assay for the direct detection of Borrelia burgdorferi in patient specimens which relies upon in vitro amplification of a 276 base pair fragment of the organism's flagellin gene. These amplified DNA segments are then detected using an internal oligonucleotide probe that has been 5' end-labelled with 32P. We are presently evaluating this assay, in comparison to the culture of spirochetes using both BSK II and MKP medium. We present data on skin biopsies from 4 groups of patients presenting with symptoms and signs of Lyme borreliosis: classical EM, status post EM, atypical/partially treated EM and classical ACA. Our results suggest that sample bias may be a significant factor in determining the relative proportions of positive and negative results. We find that PCR increases the overall yield of positive determinations. We have not encountered false positive results using PCR. We conclude that PCR is a valuable adjunct to culture for direct detection of B. burgdorferi in patient specimens.
