Abstrakt     Polycystic ovarian syndrome and pregnancy. Polycystic ovary syndrome (PCOS) is associated with a significantly increased risk of gestational diabetes, pregnancy induced hypertension, preeclampsia and delivery with cesarean section. Premature deliveries, perinatal mortality and admission to the neonatal care unit is expected more frequently after PCOS pregnancy. At the same time, the presence of increased body weight and PCOS is common among the population of infertile women. Due to the condition (infertility) and appearance (obesity) these patients are exposed to great psychological pressures from the environment as well as psychological problems related to their self-respect. In these women decide to undergo fertility treatment, they can expect suboptimal results. The mere presence of PCOS indicates a relatively high risk for the occurrence of ovarian hyperstimulation syndrome, which can threaten the patient's life. Such patients are often undesired at centers for treating infertility. Increased body-weight by itself is a sign that the fecundation is significantly decreased. However, despite the unfavourable circumstances for treatment and risk during pregnancy, such patients should not be characterized as less suitable for treatment. Evidence of increased risk for sponatneous abortion in patients with PCOS is poor, but it seems that it is more related to obesity than PCOS. Overweight women with PCOS are at greater risk for developing gestational diabetes and hypertension due to pregnancy. Equally frequent is preeclampsy, premature birth, and prenatal mortality. Although there is no evidence obtained from controlled clinical trials, it seems that giving advice and prevention of diabetes prior to and during pregnancy in women with PCOS is sensible. Reducing body-weight before pregnancy is related to better antenatal and postnatal mental health as well as better control of complications in late pregnancy and birth. Many ART centers wish to protect their reputation with good statistical results of treatments with IVF procedure by refusing to treat patients with increased body weight. PCOS, of course, presents an additional reason why these patients are denied treatment due to the possible occurrence of the most difficult complication hyperstimulation.
