Abstrakt     Uvod Poznavanje vzrokov zakaj se napake pri dajanju zdravil v zdravstveni negi ne sporočajo je izhodišče za izboljšave v praksi zdravstvene nege in nadaljnjo raziskovanje. Zato smo z raziskavo želeli ugotoviti vzroke nesporočanja napak pri dajanju zdravil. Metode: Opravili smo presečno opazovalno raziskavo. Podatke smo zbrali z uporabo standardiziranega anketnega vprašalnika. V raziskavo je bilo vključenih 91 zaposlenih na kirurških ali internih oddelkih v dveh zdravstvenih ustanovah. Za predstavitev rezultatov smo uporabili opisne statistične metode. Rezultati Zaposleni v zdravstveni negi zaznavajo, da se napake pri dajanju zdravil v zdravstveni negi ne sporočajo zaradi osredotočanja na posameznika namesto na iskanje vzrokov za napake v sistemu (x % = 4,57), obtoževanja medicinskih sester za posledice napake (x % = 3,94) in strahu pred negativnim odzivom pacienta ali njegove družine (x % = 3,76). Diskusija in zaključek Individualni pristop obravnave napak z osredotočanjem na posameznika in obtoževanjem predstavlja najpomembnejšo oviro sporočanja napaqk pri dajanju zdravil.Introduction Knowing the reasons why medication errors are not reported is a starting point for quality improvements and further research. Therefore, the aim of this study was to explore nurses% perceptions of why medication errors are not reported. Methods We conducted a cross-sectional observational study. Data were collected using a standardized questionnaire. The study included 91 employees in nursing at the surgical or medical wards in two health care institutions. Descriptive statistics were used for data presentation. Results Nursing employees perceive, that medication administration errors are not reported due to the focus on the individual rather than looking for causes of errors in the system (x % = 4,57), blaming nurses for the consequences of errors (x % = 3,94) and the fear of a negative response of the patient or his family (x % = 3,76). Discussion and conclusion Individual approach in managing medication errors, where there is an emphasis on focusing on individuals and blaming them for the consequences, is the most important reason why medication errors are not reported.
