Abstrakt     Objective: The aim of this study was to determine the volume and vascularization of tumors and peritumorous tissue by three-dimensional (3D) power Doppler ultrasound in patients with oral cavity tumors. Methods: Three-dimensional (3D) power Doppler ultrasound investigations were performed preoperatively in 49 patients with oral cavity carcinomas with a Voluson 530 device from Kretz company (Zipf, Austria). Digitally-stored data were analysed with the VOCAL-Imaging program (Virtual Organ Computer-aided AnaLysis). By using VOCAL, the borders of a astructure (tumor) can be determined and its volume calculated. The vascularization of tumors and peri-tumorous tissue was determined by analysing three-dimensional colour histograms where the number of colour voxels (the smallest unit in threedimensional space) and their grey-scale intensity were counted, representing vessels and flow. The vascularization was defined by three indexes: VI-vascularization index, which represents blood vessels; FI - flow index, which represents flow; and VFI-vascularization-flow index, which is a product between VI and FI. The vascularization was determined in 4 mm and 7 mm peri-tumoral layers, which were defined by the VOCAL program. Results: The average volume in stages T1 and T2 was almost equal - 4.5 and 5.21 cm(3). The average volume in T3 was 23.36 cm(3) and was much larger than in T4 -- 18.09 cm(3). Volume differences varied significantly between T1 and T4, T2 and T3, and between T2 and T4 (p<0,05). The tumor and peri-tumor tissue vascularization was higher in tumors with neck metastases than in tumors with no metastases. The differences between vascularization indexes in N+ and N0 neck were statistically significant. The probability value p was <0.01 in VI-4, VI-7 and VF-7. In VI-tu and VFI-tu it was <0.05. Flow index (FI) values were not statistically different between N+ and N0 neck. The difference between vascularization indexes of peri-tumoral tissue and tumor (f.e. VI-4 - VI-tu) was significantly higher in N+ than in N0 neck. Probability values were significant in VI and VF. Conclusions: The volume of oral cavity tumors and their vascularization could be determined effectively by three-dimensional power Doppler ultrasonography. Vascularization index VI, which defines the vessels inside a tumor and the peri-tumoral tissue, and the difference between peri-tumoral and tumoral vascularization, was significantly higher in patients with neck metastases.
