Abstrakt | Background. Polymerase chain reaction (PCR) bas proven to be extremely important in the study of human immunodeficiency virus (HIV) infection and AIDS and bas demonstrated both diagnostical and research utility. Recently, a standardised PCR kit (Amplicor HIV-1 DNA Test) for detection of HIV-1 DNA in blood samples was developed by Roche Molecular Systems. Material and methods. To evaluate the reliability and feasibility of Amplicor HIV-1 DNA Test a total of 190 EDTA-whole blood samples from 174 patients from Slovenia were investigated and divided into three group A comprised 36 anti-HIV-1 seropositive subjects at various stages of disease; group B comprised 18 subjects with indeterminate Western blots according to American Red Cross criteria, and group C comprised 120 comprised 120 anti-HIV 1+2 seronegative subjects. Results. All 36 anti-HIV positive, two anti-HIV indeterminate (strong p24 band, and strong gp 160 band, respectively) and one anti-HIV seronegative sample tested positive in initial testing. After retesting procedures both initially PCR-positive/anti-HIV indeterminate samples were considered true positives. After the same procedure PCR-positive/anti-HIV negative sample was considered as false-positive in initial testing. The false-positivity is most likely the result of splasbing due to wasbing procedures. Conclusions. The Amplicor HIV-1 DNA test has a good specificity and sensitivity in detection of HIV-1 infection. | |
Izvleček | Izhodišča. Številne raziskave so dokazale veliko uporabnost verižne reakcije s polimerazo (PCR) v diagnostiki in raziskavah okužbe z virusom človeške imunske pomanjkljivosti (HIV). Družba Roche Molecular Systems je nedavno razvila pri standardizirani diagnostični komplet (Amplicor HIV-1 DNA Test) za dokazovanje navzočnosti DNA HIV-1 v vzorcih polne krvi, ki temelji na PCR. Material in metode. Da bi ugotovili uporabnost diagnostičnega kompleta Amplicor HIV-1 DNA Test v slovenskem prostoru, smo z njim pregledali 190 vzorcev polne krvi (zEDTA), odvzetih pri 174 osebah, med katerimi je bilo 36 anti-HIV-1 pozitivnih, 120 anti-HIV-1/2 negativnih in 18 s trenutno neopredeljivim rezultatom metode Western-blot. Rezultati DNA HIv-1 smo dokazali pri vseh 36 anti-HIV-1 pozitivnih in pri eni anti-HIV-1/2 negativni osebi. Ponovno testiranje istega anti-HIV-1/2 negativnega vzorca ter novega vzorca, odvzetega pri isti osebi mesec dni kasneje s testom Amplicor, je v obeh primerih dalo negativni rezultat. Na osnovi dobljenih rezultatov smo rezultat prvega testiranja opredelili kot lažno pozitiven. DNA HIV-1 smo dokazali v dveh izmed 18 vzorcev s trenutno neopredeljivim rezultatom metode Western-blot. Na osnovi dodatnih kliničnih in laboratorijskih ter ponovnega testiranja obeh vzorcev smo rezultate prvega testiranja opredelili kot resnično pozitivne. Zaključki. Amplicor HIV-1 DNA Test je pokazal visoko občutljivost in specifičnost v odkrivanju okužbe s HIV, vendar bo po našem mnenju ostal rezerviran samo za razreševanje določenih diagnostičnih problemov, ki jih z drugimi metodami ne moremo dovolj dobro razjasniti. | |
