Abstrakt     The ultrastructure of glomerular extracellular matrix has not been researched in depth as yet. Our systematic research on the material of 13 biopsies of healthy human kidney proved for the first time - in accordance with very few reports on the topic - that fibrils of 3-5 nm thickness can be found in the mesangial matrix and basal membrane (according to our findings, primarily in the lamina densa) of a healthy glomerulus. The fibrils are supposed to consist of collagen IV. In mesangial matrix and in the internal lamina rara of basal membrane, very few primary tubules, which are bent, 10-12 nm thick, and of unknown origin, can be shown. We have systematically studied the increase of the number of these fibrillary structures, and found it associated not only with diabetic kidney involvement, but also with many other sclerotic processes in the glomerulus, also with those following deposition of different serum constituents. Fibrils, predominantly straight, of 15-30 nm in diameter, with subtile cross-striation, are supposedly associated with collagen III. According to our findings - and in disagreement with some other reports and the idea of collagen III being produced in the mesangial cellular cultures - they are not a normal constituent of the matrix; they seem to appear in very rare and special pathological conditions. We found that the tubules, mainly bent and 20-35 nm thick, are quite often present in the mesangial matrix and glomerular basal membrane; most likely they emerge from the surrounding cells cytoplasm and most often in diabetes patients. We proved that structura lly the most homogeneous group of the fibrils being deposited in the extracellular matrix, are the amyloid fibrils of 7-10 nm in diameter. In specimens, directly fixed with osmium tetroxide solution and embedded in Epon, the amyloid fibrils neither show cross striation nor tubular structure; they both have reportedly been found - but in conditions of preparation, different from ours.(trunc.)
