Abstrakt     Etiology and pathogenesis of essential arterial hypertension is not known yet. Abnormalities found in patients could be related to pathogenesis but maybe they are only the consequences of longstanding elevated blood pressure. In prospective population study in Ribnica, using epidemiological methods, we wanted to find clinical and laboratory data that are characteristic for the group of individuals with higher risk for developing hypertension. These data could support the neurogenic hypothesis of etiopathogenesis of hypertension and show us the predictive factors for future hypertension. The aim of our study was to compare our data with data from similar study going on in Tecumseh (USA) too, concluding how important the environment and way of life are on the pathogenesis of hypertension. In the study 477 healthy individuals were included, 251 with high risk for developing hypertension and 226 with low risk, Both groups were comparable and there was clear difference in blood pressure (systolic and diastolic) with higher blood pressure in high risk group, but surprisingly there were no differences in heart rate as a marker for hyperadrenergic state proposed in neurogenic hypothesis of hypertension. Neither reactivity, white coat phenomenon, nor laboratory data support the hypothesis. We found elevated minimal forearm peripheral resistance in the group of high risk indicating that structural changes could be initiating factor in pathogenesis of hypertension. Further observation of the individuals is needed to confirm our data.
