Abstrakt     The authors present the possibilities of continuous monitoring of cerebral blood flow in the middle cerebral artery by transcranial Doppler sonography during carotid endarterectomy. They present the monitoring of changes in intracranial haemodynamics during individual phases of operative treatment of carotid stenosis in the neck. In this way it is possible to follow any changes in cerebral blood flow, especially during the individual critical phases of the operation, such as clamping of the branches of the carotid trunk and applying intraluminal shunt. Such continuous transcranial Doppler sonographic examination during the operation gives us important data on the successfulness of collateral cerebral circulation and on the successfulness of the intraluminal shunt. It enables the early detection of microembolism in the intraluminal shunt and discloses the need for its immediate revision. The possibility of the early detection of threatening cerebral arterial spasm and immediate adequate drug treatment is also of extreme importance. The authors also present their first experiences in the introduction of the transcranial Doppler sonographic monitoring of cerebral circulation during carotid endarterectomy.
