| Abstrakt | Laparoscopic cholecystectomy is the most common surgical procedure in Slovenia as well as worldwide. Its integral part is cholangiography, which delineates the course of bile ducts, identifies anatomic variations and anomalies, prevents ductal injury and rules aout the presence of ductal stones. Many surgeons are still convinced, however, that because of problems with cystic duct cannulation, the failure rate of cholangiography during laparoscopic cholecystectomy is hight. In a prospective study we evaluated the feasibility of transcystic-duct cholangiography using different surgical procedures on the cystic duct in our patients undergoing laparoscopic cholecystectomy. Twenty-four female and 6 male patients with a mean age of 49 years (range 26-65) were studied. The cholangiocatheter was introduced into the peritoneal cavity the middle subcostal trocar and advanced into the biliary tree via an anterior transverse or oblique ductotomy, or via a dilated or longitudinally incised cystic duct. Cholangiography was "feasible", if the catheter was succesfully introduced into the cystic duct and the contrast medium was injected without difficulty the biliary tree. Transcystic cholangigraphy was feasible in all the patients studied. In 17 patients (57 percent) the catheter was introduced via a transverse or oblique ductotomy, in 8 (27 percent) via a previously dilated cystic duct, and in 5 (16 percent) via a ductotomy widened by longitudinal incision. Our results confirm that during laparoscopic cholecystectomy feasibility of transcystic-duct cholangiography using different surgical procedures on the cystic duct is high. | |
