Abstrakt     The paper presents certain characteristics of people with mental disorders which should be taken into account when formulating policies of rehabilitation and maintenance of health of this population group. The starting point is "the vulnerability stress model" of schizophrenia, which enables us to understand the close relationship between psychological, social and biological elements of psychotic illness, as well as certain consequences of mental disorders. The authors describe the management of mental disorders provided within the present health and social care system, and call attention to problems commonly encountered by the patient's relatives and foster parents. Next, various factors affecting health, morbidity and psychosocial status of this population are dealt with. These include: in-patient treatment, untoward effects of long hospital stay and the role of environmental factors which either trigger the recurrence of illness, or maintain health, and thus obviate the need for acute medical interventions. THe paper presents the out-patient services for the mentally disordered, and stresses the role of the primary health care sector. Proposals for the improved organization of in-patient and out-patient services are sugested. An important policy goal should be to support the setting up of community-based services for mental health, and to evaluate their work.
