Abstrakt     Backgraund. We describe a new liver function test, based on the formation of the lidocaine metabolite monoethylglycinexyilidide (MEGX). MEGX is formed from lidocaine via oxidative N-deethylation by the hepatic cytochrome P 450 system. The aim of our study was to determine the utility of MEGX test in comparison with standard liver test (SLT) to assess hepatic functional reserve in children with chronic liver disease. Patients and methods. From January 1933 to April 1996 we made MEGX test at 22 children with chronic liver disease. MEGX test was performed by slow intravenous injection of lidocaine (1 mg/kg) and measurements of its principal metabolite, MEGX were determined in serum. 16 children from 3 months to 18,5 years were enrolled in our study. SLT were made to all children. We excluded the children who haven't had SLT. According to MEGX test we devided all patients into 2 groups: 1. 9 children with chronic liver disease with still normal MEGX test (a normal concentration was defined as equal or more 34 micro g/L). This group encluded patients with extrahepatic biliary atresia or hypoplasia (4), neonatal hepatitis (1), Alagille's syndrome (1), cystic fibrosis (1), hepatic and renal polycystic disease (2). 2. 7 children with hepatic cirrhosis with pathologic MEGX test (< 34 micro g/L). This group encluded patients with extrahepatic biliary atresia, hypoplasia or agenesia (5), cystic fibrosis (1), chronic hepatitis (1). Statistical analysis was made using Wilcoxon's range sum test. Resutls. Serum concentration of cholinestterasis was statistically lowered (p<0,05) in the group of children with pathologic MEGX test. It was the same with the higher level of ammonia in the seerum.
