Abstrakt		Calciphylaxis is a syndrome of progressive vascular and soft tissue calcification and ischemic skin necrosis associated with secondary hyperparathyroidism (SH). The clinical course in four patients (3 women, 1 man; aged from 46 to 70 years) with calciphylaxis treated in our institution were reviewed. All had painful ulcers, severe hyperphosphatemia and in only one patient serum parathyroid hormone levels were severely elevated (i-PTH 1019 micro g/L). In all patients, skin biopsy specimens of the involved area showed extensive cutaneous necrosis with calcification of the vessels without signs of vasculitis. The treatment of calciphylaxis consisted of local wound are, abtibiotics, phosphate-binding agents and parathyroidectomy. Three oatuebts with extensive necrosis died due to uncontrollable sepsis. The only patient that survived had parathyroidectomy done. Calciphylaxis is a life-treatening complication of secondary hyperparathyroidism. Early diagnosis remains of vital importance since parathyroidectomy and lowering of phosphorus could be life saving in some cases.
