Abstrakt     The correlation between left ventricular mass (LMV) and arterial hypertension has been found in only 25% of hypertensives. To evaluate whether a nonhemodynamic factor such as insulin influences heart muscle mass, we studied 46 nondiabetic, middle aged hypertensives (SBP 150 +- 11, DBP 97 +- 6 mm Hg); 21 healthy volunteers served as the control group. Only men were included. We measured blood pressure and calculated Quetelets index. Fasting insulin, glucose, and lipid concentrations were determined. Fasting insulin concentration was determined by radioimmunoassay. A bidimensional echocardiographic analysis was performed on every enrolled subject. Measurements of posterior wall thickness, interventricular septal thickness, and left ventricular diastolic dimensions were made. LVM was calculated according to the method of Devereux. We also calculated LVMI and LVM/Ht. Statistical analyses were performed using the multiple linear regression method. Fasting insulin concentration in the group of hypertensives was 12.3 += 14.7 micro U/ml and in the control group 5.6 +- 5.1 micro U/ml (p < 0.05). LVM in the group of hypertensives was 237 +- 87 and in the control group 188 +- 45 g (p < 0.02). We found a positive correlation between DBP and fasting insulin concentration in the group of hypertensives. No correlation has been found between SBP and fasting insulin concentration. We cannot conclude that insulin concentration. We cannot conclude that insulin is responsible for left ventricular hypertrophy in patients with hypertension. The results of our study show that the insulin levels in hypertensive patients are significantly higher than those in normotensives, but still normal. The growth effect of insulin might have a role in left ventricular hypertrophy only in hypertensive patients with hyperinsulinemia.
