Abstrakt     Patients with replanted fingers often develope cold intolerance, the mechanism of which is poorly understood. To further clarify the components of microvascular response to cooling we simultaneously performed Laser-Doppler (LD) flowmetry on tips of replanted fingers and on contralateral normal fingers in 10 patients, before and during exposure to cold water at 15 degree C. During the direct cooling of normal fingers average LD flow (N=10) decreased significantly to 64.6+-6.6% of the resting flow while the cooling of normal fingers on contralateral hand caused the reduction of flow to 81.9+-5.3% of the resting flow. Direct cooling of replanted fingers caused significant increase of average flow to 107.8+-7.2% of the resting flow. These results are consistent with the hypothesis that the risk of pathologic vasospasm, which eventually develops in replanted fingers, is probably associated with vascular reinnervation and concomitant disregulation of the vascular sponse.
