Abstrakt | Background. In patients with end-stage renal failure extraskeletal calcifications may be found in blood vessels, cornea and conjunctiva, periarticular tissue, skin and in internal viscera. Ectopic calcifications can also develop in the native kidneys in hemodialysis (HD) patients during renal replacement therapy. Methods. Fifty-nine end-stage renal failure patients treated with maintenance HD and 59 control patients with normal renal function were included in the study. The presence and number of kidney calcifications in each HD patient were established with real-time ultrasonography. The calcifications' volume was calculated. The degenerative kidney lesions (acquired cystic disease of the kidney) were also established. Serum levels of calcium (Ca) (total and ionized), phosphate (P), parathyroid hormone (PTH) and aluminum (AI) were regularly measured. Average and maximal Ca x P product was calculated. The possible association between these biochemical parameters and kidney calcifications was investigated. The presence of extraskeletal calcifications in blood vessels, periarticular tissue (with X-rays) and cornea and conjunctiva (with slit-lamp) were established. The possible association between these ectopic calcifications and kidney calcifications was also investigated. Results. Kidney calcifications were found in 28 (47,5%) HD patients and just in 1 (1,7%) control patient. HD patients with kidney calcifications were treated longer with HD, they had higher maximal Ca x P product and higher PTH serum levels. The degenerative kidney lesions were also found more frequently in these patients. No association was found between kidney calcifications and serum levels of Al.
