Abstrakt     Venous occlusive disease (VOD) is a frequent underlying condition in the organically caused male impotence. The data from the literature show variable degree of success of therapy of this condition. therapy is less successful in diabetic patients. Into the frst part of our study we included 200 males who had a permanent heterosexual partner and had suffered from impotence of organic cause for at least 6 months. Less than one half of the patients (90) were diabetics. Out of the 200 we selected 70 patients with erectile dysfunction due to venous occlusive disease. Using certain criteria we included into our study 40 patients with absent erection due to venous occlusive disease with venous leakage diagnosed with cavernosometry after the exclusion of other causes of impotence. All the 40 patients had pathological venous drainage mainly through the deep dorsal penile vein which was proved with cavernosography. One half of the patients were diabetics. We compared the results of two methods of treatment: embolisation of the DDPV system and use of the vacuum apparatus. In addition, the results of both methods of treatment in diabetics were compared with the results in non-diabetics. The groups did not significantly differ as to the age and duration of impotence. The patients were followed up for 9 months. We did not advise them as to the frequency of intercorse, and they could make use of intracavernous injections of a vasoactive drug. Every fortninght we recorded the occurrence of erections and the frequency of successful intercorse. Using number of points, the patients described their present satisfaction with sexual life and so made their subjective evaluation of the success of the treatment. 45% of patients treated with embolisation of the DDPV system had spontaneous erections, and 65% in case they used intracavernous injections of a vasoactive drug.
