Abstrakt     Background. There have been numerous studies concerning the diagnosis, treatment and prognosis of patients with papillary thyroid carcinoma, but only few addressing patients with follicular thyroid carcinoma. The aim of this retrospective study was to find out how the extent of surgery influences disease free interval and recurrence rate and which factors influence disease free interval and survival. Material and methods. The patients with folicular thyroid carcinoma treated at the Institute of Oncology in Ljubljana from 1972 to 1992 were included inour study. In the study of disease free interval and recurrence rate, there were 92 patients (70 women, 22 men; median age 55 y.) with no residual tumor after operation and, in the study of prognostic factors all 154 patients (113 women, 41 men; median age 61 y.) with follicular carcinoma were included. We collected data about: gender and age of patients, type of treatment, recurence, disease free interval and survival. The relationship between various factors and the length of disease free interval and of survival was analysed using univariate and multivariate (Cox's) statistical methods. Results. The probability for longer disease free interval after total or near-total thyroidectomy was, 1,49-times higher than after lobectomy and 3,98-times higher than after subtotal thyroidectomy. There were no statistical differences inrecurrence rates after total and near-total thyroidectomy incomparisonwith lobectomy. However, there were more recurrences after subtotal thyroidectomy in comparison with more extensive. Factors that influenced disease free interval beside the extent of surgical operation and ablation with radioiodine were: age, confinement of tumor to the thyroid and type of macroscopic tumor growth.
