Abstrakt     The role of a diet in provoking an acute arthritis in a gouty patient has been well known for centuries. Many gouty patients are overweight, therefore the restoration of ideal weight though caloric restriction is to be strongly advocated. Some gouty patients respond to gradual weight loss with a reduction in the serum urate concentration and urinary uric acid excretion. The dietary indiscretions of holidays are especially prone to end in a gouty arthritis. The patients should be instructed about the deleterious effects of alcoholic beverages and a diet with high fructose content, which increase the production of uric acid. It is reasonable to avoid an excessive intake of food that contains high amount of purines in average gouty patients. A diet restricted in purine content will reduce the urinary excretion of uric acid by 200 - 400 mg per day and lower the mean serum urate value for about 50 micro mol/1. In some patients the reduction is even greater. A severe dietary purine restrictions is usually not necessary, unless renal excretory function is very poor and the control of hyperuricemia with drugs is unsatisfactory.
