Abstrakt     It was the intention of the present study to find out whether in patients with documented coronary heart disease improvement of contraction of poststenotic noninfarcted hypokinetic left ventricular segments (PNHS) 30 min after oral intake of calcium antagonist nicardipine is indicative of improved contraction of these segments 6 months after successful PTCA of related artery stenosis. in 18 PNHS of 20 patients a close correlation (r = 0.82; p < 0.001) was found between post-nicardipine (22 +- 4% 40 +- 8%; p < 0.00005) and post-PTCA (22 +- 4% 35 += 8%; p < 0.0001) improvement of mean relative contraction amplitude using quantitative left cineentriculography and hemaiaxis method (x+ gama r %) for wall motion analysis. The behaviour of PNHS can acordingly be assessed as typical for hibernating or chronically but reversibly dysfunctioning myocardium. The effect of PTCA on the contraction of these segments can obviously be predicted by the effect of calcium antagonist nicardipine. Minor but significant improvement of contraction was found even in poststenotic infarcted and in poststenotic noninfarcted normokinetic segments after nicardipinise as well as after PTCA, indicating the presence of hibernanting myocardium.
