Abstrakt     Background. Despite using numerous diagnostic procedures, preoperative differentiation between various ovarian tumors is difficult. During the years color Doppler ultrasound, which enables us to study tissue vascularization, has been added to the already established gray scale ultrasound diagnostics. The role of color Doppler ultrasound in preoperative determination of the nature of ovarian tumors is still not exactly determined. Methods. In a prospective study we tried to determine the accuracy of color Doppler ultrasound in distinuishing between benign, malignant and ovarian pseudotumors. Prior to surgery, pulsed color Doppler examination was performed in 40 patients with benign, 40 with pseudo- and 40 with malignant ovarian tumors. Vascularization of the tumors as well as their adjacent surroundings and blood vessels location was studied using color coded Doppler frequency shift analysis. Spectral analysis was used to study vascular blood flow, expressed with the resistence index RI. Results. Vascularizaton was equally frequent in all three groups of tumors. Only in rare tumors vascularization was absent. Blood vessles of benign and pesudotumors more often had a tumoradjacent or peripheral intratumoral location, while malignant tumor vessels exhibited more often a diffuse, intrasptal or intraprofireation location. RI values of benign and pseudotumors were significantly higher than those of malignant tumors. The mean value of RI+-SD was 0.56+-0.15 in benign, 0.55+-0.12 in pseudotumors and 0.33+-0.13 in malignant turmors. The differences in RI between benign and malignant tumors as well as between pseudotumors and malignant tumors are statistically significant (p < 0.01). In detecting malignant ovarian tumors, the sensitivity of RI <- 0.40 is 82%, its specificity 97%, the positive predictive value 94% and negative predictive value 91%.(Astract truncated at 2000 characters.)
Izvleček     Izhodišča. Kljub uporabi številnih diagnostičnih metod je preoperativno razlikovanje med različnimi vrstami tumorjev jajčnikov težavno. Že docela uveljavljeni ultrazvočni diagnostiki v sivi skali se je v zadnjih nekaj letih pridružil barvni Dopplerjev ultrazvok, s katerim lahko prikažemo ožiljenost posameznih organov. Vloga barvnega Dopplerjevega ultrazvoka pri preoperativnem določanju narave tumorjev jajčnikov za enkrat še ni povsem pojasnjena. Metode. V prospektivni raziskavi smo skušali ugotoviti zanesljivost barvnega Dopplerjevega ultrazvoka pri razlikovanju med benignimi, nepravimi in malignimi tumorji jajčnikov. Pred operacijo smo s pulzirajočim barvnim Dopplerjevim ultrazvokom preiskali 40 bolnic z benignimi, 40 z nepravimi ter 40 z malignimi tumorji jajčnikov. Z analizo pomika Dopplerjevih frekvenc, kodiranega v barvi, smo ugotavljali ožiljenost tumorjev in njihove neposredne okolice ter razporeditev žil v njih. S spektralno analizo smo ugotavljali pretok krvi v žilah in ga izrazili z rezistentnim indeksom RI. Rezultati. Vse tri skupine tumorjev so bile enako pogosto ožiljene. Le redki tumorji niso bili ožiljeni. Žile benignih in nepravih tumorjev so bile pogosteje nameščene v okolnem tkivu ali na periferiji tumorjev, malignih padifuzno v tumorjih, v septih in proliferacijah. Benigni in nepravi tumorji so imeli značilne višje vredosti RI od malignih tumorjev. Srednja vrdnost RI+-SD benignih tumorjev je bila 0,56+-0,15, nepravih 0,55+-0,12 ter malignih 0,33+-0,13. Razlike med RI benignih in malignih ter nepravih in malignih tumorjev so statistično značilne (P < 0,01). Pri ugotavljanju malignih tumorjev jajčnikov ima RI <- 0,40 občutljivot 82%, specifičnost 97%, pozitivno napovedno vrednost 94% in negativno napovedno vrednost 91%.(Izvleček prekinjen pri 2000 znakih.)
