Abstrakt     Isometrical torque in the ankle was measured intraoperatively at L5 root monopolar electrical stimulation in 11 patients (38yrs +- 3,4) with verified disc herniation by mielography after 4-8 months of unsuccessful treatment for radicular painsyndrome. Only very mild or absent motor deficits or electrophysiological abnormalities were qualified for the inclusion criteria. A multipolar cuff electrode was wrapped around L5 spinal root at the axilla of dural sleeve after root decompression. Evoked potentials from the crural muscles and 3D torques from the ankle were recorded at each position (N=8-9) of the cathode around the root circumference. All L5 roots produced plantar flexion on stimulation, but the responses of the y and z axes were more variable. Recruitment curves for each of the electrodes had shifted laterally so that the torque ratio between some of the two adjacent electrodes was up to 1:3. Average torque on plantar flexion (N=11) was 9,2 Nm +- 3,2. As the normal response to L5 root stimulation was plantar flexion despite evident submaximal cocontraction of peroneal muscles, the motoneurons for dorsiflexors are anticipated to be in a minority in L5 roots and are distributed in such a way that they are not accessible to selective activation by the afore mentioned monopolar stimulation. It may be concluded that the L4 root is the proper root for foot dorsiflexion.
