Abstrakt     A series of 710 patients with meningiomas in the cavernous sinus (CS) and adjacent areas have been surgically treated by the author over the past 15 years. As the philosophy of treatment of skull base meningiomas has constantly been changing in the last two decades, the surgical technique has also made some major advances. The latest version of the surgical approach to centrall skull base meningiomas represents a cutting edge in the surgical treatment and has very little in common with the approach to the CS used initially (2). The success rate of resection of skull base meningiomas in the presented series was: total resection in 405 patients (57%), gross total in 149 patients (21%), subtotal in 99 patients (14%), and partial in 57 patients (8%). The main surgical complication was CSF leak which occured in 35 patients (5%). A temporary loss (partial or complete) of function of the oculomotor nerve(s) was present in 575 patients (81%) one wek after surgery, sixth months after surgery it persisted only in 35 patients (5%), and the end of the first postoperative year in 14 patients (2%). The tumor recurrence rate was none in those cases where total or gross total resection was achieved. The residual tumor in patients with subtotal or partial removal of the tumor showed growing in most cases, and became noticeable after the first postoperative year and a reoperation was required in 20 cases.
