Abstrakt     The degree of angiogenesis in human tumors can predict the likelihood of metastatis. This was first demonstrated in malignant melanoma of the skin. Later this hypothesis was confirmed in other tumors, i.e. breast cancer, non-small carcinoma of the lung, carcinoma of the bladdr, gastric carcinoma and squamous cell carcinoma of the head and neck. The correlation between angiogenesis and the probalitiy of regional metastasis has been demonstrated also in cancer of the oral cavity, where microvessel density (MVD) is useful also as an independent prognostic factor. However, in an investigation of squamous cell carcinoma of the lower lip, the authors found no significant association between angiogenesis and metastatic spread. In the present study, the hypothesis that MVD, an index of tumor angiogenesis, correlates with regional metastasis was tested in two groups of patient with invasive squmaous cell carcinoma of the lower lip. Group 1 comprised 23 patients with regional metastasis and group 2 consisted of 21 patients without metastasis. MVD was determined by counting immunohistochemically identified microvessels per unit of tumor surface in areas of the greatest neovascularization. Any clearly separate brown stained endothelial cell or cluster of endothelial cell was counted as one microvessel. Univariate analysis of the MVD data revealed a statistically significant difference between group 1 and group 2 (p = 0.042), and multivariate analysis showed MVD to be an independent prognostic factor for regional lymph node metastasis (p = 0.043). No correlation was established between MVD and the stage of the primary tumor, although tumor stage is a stronger prognostic factor for metastasis than MVD (p < 0.001). The study showed that in patients with carcinoma of the lower lip, the likelihood of metastasis increases in proportion with MVD.(Abstact truncated at 2000 characters.)
