Abstrakt | Until the early 60's staphylococcal penicillinase has been the only one known beta-lactamase. Between 1960 and 1978 plasmids encoding beta-lactamase (e.g. TEM-1) have spread amond Gram negative bacilli due to increased consumption of broad spectrum penicillins and older cephalosporins. In the last decade extended-spectrum beta lactamases (ESBL) have emerged as a special problem. They have a special affinity for 3rd generation cephalosporins and monobactams. They've been predominantly detected in stains of Escherichia coli and Klebsiella pneumoniae. At Clinic for Respiratory Diseases and Allergy Golnik we used cephpodixine disk as a screening method for ESBL producers. From clinical specimens we've isolated 95 E. coli and 18 K. pneumoniae strains. We performed a disk diffusion susceptibility testing on all strains using cephpodoxime, cephotaxime, cephtriaxone, cephtazidime and aztreonam disks and a commercial Etest ESBL screen test (AB Biodisk, Solna, Sweden). We detected only one ESBL producing E. coli strain. Inspite a low precentage of ESBL producing strains among isolates in our hospital )1%), searching for ESBL producers is imperative. | |
