Abstrakt     Focal segmental glomerulosclerosis (FSGS) is considered one of the most important recurrent diseases after transplantatio due to the frequency andseverity of the disease. Thirteen patients (5,8%) developed end stage renal disease as the result of bipsy proven FSGS in native kidney. Seven patients (all 4 from living and 3 from 9 cadaver related donors) developed recurrent FSGS. Thirty-two renal transplant biopsies and 3 nephrectomy specimens were obtained from these patients. All biopsies were taken for the clinical reasons either proteinuria and/or increased s. creatinine due to suspected rejection or cyclosporine nephrotoxicity. In early allograft biopsies (5 days to 6 months) minimal change nephropathy (6 biopsy specimens) or mesangial hyperplasia (4 biopsy specimens) were demonstrated. In five patients with proteinuria only after repeated transplant biopsies (3 - 14 months after transplantation) typical FSGS was seen. In patients with FSGS and proteinuria after transplantation minimal change glomerulonephritis and mesangial proliferation in the absence of other lesions are indicative of recurrent FSGS.
