Abstrakt | The ultrastructure of the interstitial capillary basal lamina (BL) was studied in119 patients with renal allograft rejection (25 biopsies), Balkan endemic nephropathy (15 biopsies), syndroma Alport (37 biopsies), diabetic glomerulopathy (23 biopsies) and Hantan nephropathy (19 biopsies). The control group consisted of 15 biopsies of normal kidney. Thickening with diffuse multilayering of BL was found to the most intensive in patients with transplant glomerulopathy, and much less intensive in patients with acute tubulointerstitial rejection and in patients with recurrent or de novo glomerulonephritis. Though rather specific, these changes are not pathognomonic for transplant glomerulopathy, since interstitial capillary of our patients with Balkan endemic nephropathy showed similar BL thickening and splitting. Interstital capillary BL changes can be quite similar to glomerular BL alterations in transplant as well as in diabetic glomerulopathy, so that a common cause and mechanism can be presumed for lesions on both locations. However, lesions can be found isolated either only to glomerular BL (sindroma Alport) or to intersitial capillary BL (Hantan nephropathy, acute tubulointerstitial rejection). | |
