Abstrakt     From the history of children health care in Slovenia it can be seen, how programs, activities and institutions in this field changed with changes of technologies and social values. However, health goals (maximal health, maximal quality of care and maximal accessibility of services) remained the same. This developmental view is also the perspective in which the school medicine can be evaluated. Because of the biological and social characteristics of young people the classical schoolchildren health indexes do not comprise all health needs of this population. Health effects, caused by heterogenous and intensive risk factors to which schoolchildren are exposed, do not become obvious before the adult age. This characteristic of problems requires in first place the detection of the risk factors and preventive measures in contect of school education, interpersonal relations within family, specific needs of young people and coordination of various activities for youth. School medicine is a specific field that can manage health-related problems, if acting from the social medicine standpoints on the primary health care level. For the accomplishment of sociomedical legacy of school medicine clear national programs, determined public health competences and specially qualified physicians are required. School medicine is (not only in Slovenia) tied between the clinical pediatrics on one hand and the social medicine/public health on the other. Appropriate integral placement of school medicine inside this frame is a challenge for the Slovenian as frame is a challenge for the Slovenian as well as for the European health care and broader social policy.
Izvleček     Iz zgodovine zdravstvenega varstva otrok v Sloveniji izhaja, da so se programi, dejavnosti in institucije na tem področju spreminjale hkrati s tehnologijami in družbenimi vrednotami, toda zdravstveni cilji: čim boljše zdravje, čim višja kakovost oskrbe in čim bolj približana dejavnost so ostali nespremenjeni. V tem razvojnem smislu lahko tudi vrednotimo šolsko medicino. Zaradi bioloških in socialnih značilnosti pa klasični kazalci zdravstvenega stanja šolarjev (umrljivost, zbolevnost ipd.) ne odsevajo vseh njihovih zdravstvenih potreb. Namreč, raznoliki in intenzivni ogrožajoči dejavniki, ki so jim izpostavljeni šolarji, kažejo bolezenske posledice šele v odraslem obdobju. Takšna narava problemov zahteva predvsem odkrivanje ogrožajočih dejavnikov in preventivno ukrepanje v sklopu izobraževanja, družine, specifičnih potreb mladih in usklajevanja različih dejavnosti za mlade. Šolska medicina je specifična dejavnost, ki lahko z zdravjem povezane probleme obvladuje, če iz socialnomedicinskih stališč deluje na ravni primarnega zdravstvenega varstva. Za uresničitev socialnomedicinskega poslanstva šolske medicine so potrebni jasni nacionalni programi, določene javnozdravstvene pristojnosti in posebej usposobljeni zdravniki. Šolska medicina (ne samo) v Sloveniji je vpeta med klinično pediatrijo na eni in socialno medicino oz. javnim zdravstvom na drugi strani. Ustrezna integralna umestitev šolske medicine znotraj tega razpona je izziv za tokratni kongres, naše in evropsko zdravstvo ter širšo socialno politiko.
