Abstrakt		The history and concept of less and minimally invasive coronary artery surgery is presented. The mos timportant part of lesser invasiveness, avoidance of cardiopulmonary bypass (CPB) is stressed. A left internal mammary graft to the left anterior descending coronary artery through small anterior thoracotomy on a beating heart (LAST; MIDCABG) is ecplained in more details. Our limited experience with this procedure is presented. From December 13th 1996 to to September 31st 1998, fife patients, three men and two women, aged 50 to 59 years were scheduled for LAST operation. In two patients there was a history of unsuccessful percutaneous transluminal angioplasty and a bentall oepration for aortic regurgitation and aneurysm of the ascending aorta. The procedure was straightforward in two patients, because of a mammary artery lenght problem interposition of a piece of saphenous vein was necessary in two and in one, because of a dissection of heavily diseased coronary artery a conversion to classic operation was done. In all cases patency of the bypass was confirmed by Dopler echography. No postoperative complications were registered an early mortality was 0%.
