Abstrakt		Purpose: Because of an old severe open globe injury a 68- year- old man is on one eye blind, on the other eye visual acuity is light perception. We decide for an operative anterior segment reconstruction of the better eye. Methods: Anterior segment reconstruction is shown on the video. With a penetrating keratoplasty a large vascularized corneal scar is removed, peripheral anterior and posterior aynechiae are lysed. After removing a reduced traumatic cataract and an anterior vitrectomy the posterior IOL is implanted with sutering the haptic into sclera. Results and conclusion: With performed operative technique we can succesfull reconstruct a severe damaged anterior segment of the eye.
