Abstrakt     The aim of this clinical prospective study was to establish certain morphological and functional parameters of peripheral blood vessels by the aid of duplex sonography as noninvasive diagnostic method. The parameters measured would indicate the optimal location for construction of arteriovenous fistula for haemodialysis, the time needed for its development and other conditions for optimal function of chronic vascular access. Out of 116 patients (pts) observed 62 were female and 54 male, average age was 51.4. Due to chronic renal failure and need for haemodialysis the construction of permanent arteriovenous fistula was necessary. Foregoing surgical procedure, the arteries and veins of the forearm were clinically examined. Duplex sonography was used to assess: the internal diameter of the veins before and after compression with tourniquet to induce venous congestion, the venous Doppler signal, the respiratory filling of the vein, the internal diameter of the artery, the thickness and structure of artery wall, systolic and diastolic velocity of blood flow, resistance and pulsatility index before and after the peripheral resistance change (due to opening of the fist), arterial blood flow. During the surgical procedure, we measured the diameters of the artery and vein, the thickness of arterial wall and we observed the arterial wall structure. The results of the measurements were analysed regarding their relation to immediate fistula patency in pts with functional fistulas as well as in those with non patent fistulas. We countinued measurements in pts with functional accesses. The first sonographical examination was performed 24 hours after surgical construction, then once weekly for three weeks, again 8 weeks after, an finally 12 weeks following the construction of the fistula.
